In-Person: Visit our customer service desk located in the front lobby. Any staff member will be happy to direct you.

What Happens Next

Once your grievance form has been received, our patient relations team will review it promptly. Here’s what you can expect:

  • Your grievance will be acknowledged in a timely fashion.
  • We will conduct a thorough investigation into your concerns.
  • We will provide a written response within 30 days, outlining our findings and any actions taken.
